Penggunaan Statin pada Pasien Hiperkolesterolemia

Hiperkolesterolemia ditandai kadar kolesterol total di dalam darah melebihi batas normal (>200 mg/dL). Pedoman
National Cholesterol Education Program (NCEP), Adult Treatment Panel III (ATP III) merekomendasikan
penggunaan statin sebagai pilihan pertama pengobatan untuk menurunkan kadar kolesterol total dan LDL dalam
darah. Tujuan penelitian ini untuk melihat perubahan kadar kolesterol total, LDL, HDL dan trigliserida pasien
setelah pengobatan golongan statin. Pengambilan data dari rekam medik sejak Januari 2015 - Januari 2017 di
dua rumah sakit. Terdapat 58 kasus memenuhi kriteria inklusi, 42 perempuan dengan usia rata-rata 59 ± 9 dan
16 laki-laki dengan usia rata-rata 60 ± 9. Statin yang digunakan di dua rumah sakit adalah simvastatin (70,7%)
dan atorvastatin (29,3%). Terdapat 23 pasien dengan kadar kolesterol tinggi yang dapat mencapai nilai optimal
sesuai NCEP ATP III. Hal tersebut dipengaruhi beberapa hal diantaranya ketidakpatuhan pasien dalam pengobatan;
pertimbangan biaya obat sebagian pasien; evaluasi atau monitoring pengobatan tidak dilakukan secara berkala;
pemeriksaan kadar kolesterol tidak dilakukan secara konsisten sehingga tingkat keberhasilan terapi yang dijalani
tidak diketahui atau perlunya penyesuaian dosis atau jenis statin yang diberikan. Sebab itu monitoring dan evaluasi
terapi hiperkolesterolemia perlu dilakukan.

Abstract
Hypercholesterolemia is a condition of cholesterol levels in the blood beyond the normal limits (>200 mg/dL).
National Cholesterol Education Program (NCEP) Adult Treatment Panel III (ATP III) Guidelines recommends the
use of statins as the first-choice drug to decrease total cholesterol and LDL levels in the blood. The purpose of
this study was to looking changes in total cholesterol, LDL, HDL and triglyceride levels of patients after statin
treatment. Data collection from medical records from January 2015 - January 2017 in two hospitals. There were 58
cases of hypercholesterolemia fulfilling the inclusion criteria, 42 women with mean age 59 ± 9 and 16 men with
mean age 60 ± 9. Statin used in both hospitals were simvastatin (70.7%) and atorvastatin (29.3%). Only 23 patients
with high cholesterol levels reached optimal levels regarding NCEP ATP III. This happened due to several things,
imcompliance of patients with treatment; consideration of drug costs for some people; treatment evaluation or
monitoring is not performed regularly; examination of cholesterol levels is not done consistently so that the success
rate of therapy is unknown or required adjustment of dose or type of statins selected. Therefore, monitoring and
evaluation of hypercholesterolemia therapy needs to be improved.
