Secara histopatologi ditemukan gambaran bulan sabit atau crescent pada sebagian besar glomerulus. Gambaran klinis GNPC berupa hematuria, proteinuria, penurunan diuresis, edema, dan hipertensi. Kebanyakan GNPC mengalami gagal ginjal akut yang berhubungan dengan nefritis akut dan atau sindrom nefrotik. Perkembangan menjadi gagal ginjal stadium akhir biasanya terjadi dalam beberapa minggu hingga beberapa bulan setelah awitan. Glomerulonefritis progresif cepat terdiri atas tiga tipe yaitu glomerulonefritis kompleks imun, glomerulonefritis pauci-immune, dan glomerulonefritis anti-membran basal glomerular. Tata laksana GNPC dibagi menjadi dua bagian. Bagian pertama adalah tata laksana spesifik untuk mengatasi peradangan pada ginjal. Glomerulonefritis progresif cepat yang tidak diobati akan cepat berkembang menjadi end-stage renal disease, sehingga terapi spesifik berdasarkan penyebab harus segera dimulai meskipun bukti pendukung keberhasilan terapi masih belum memadai. Bagian kedua adalah penanganan akibat kelainan glomerulus, GNPC harus mendapatkan terapi suportif untuk mengontrol komplikasi penyakit glomerular seperti uremia, hipervolemia, hipertensi, hiperkalemia, dan asidosis metabolik. Jika diperlukan, dialisis dan ultrafiltrasi harus dilakukan. Secara umum prognosis GNPC tidak baik, bahkan setelah pengobatan dengan steroid oral dan sitotoksik. Kata kunci : glomerulonefritis akut, glomerulonefritis progresif cepat, kresentikAbstract Rapidly progressive glomerulonephritis (RPGN) is glomerulonephritis accompanied by rapid loss of renal function (usually 50% decline in glomerular filtration rate) within three months, and histopathologic abnormality of crescent formation seen in most of glomeruli. The clinical manifestations of RPGN are hematuria, proteinuria, decrease of diuresis, edema, and hypertension. Most of RPGN progress to acute kidney injury associated with acute nephritis and/or nephrotic syndrome. Progression into end-stage renal disease usually occurs within few weeks to few months after onset. Rapidly progressive glomerulonephritis (RPGN) is classified into three types; immune complex glomerulonephritis, pauci-immune glomerulonephritis, and anti-glomerular basement membrane glomerulonephritis. Management of RPGN consists of two parts. The first part is specific management to overcome inflammation of the kidney. Untreated RPGN will rapidly progress to end-stage renal disease, therefore specific therapy based on underlying causes must be initiated immediately although supporting evidences of succesful therapy are still inadequate. The second part is the management of the consequences of its pathophysiologic glomerular abnormality. A supportive therapy must be given in order to control glomerular disease complication such as uremia, hypervolemia, hypertension, hyperkalemia, and metabolic acidosis. Dialysis and ultrafiltration must be initiated if required. Generally, prognosis of RPGN is poor even after treatment with oral steroids and cytotoxic drugs.Key words : acute glomerulonephrtis, rapidly progressif glomerulonephritis, crescent ER -
